Prior versions may not be used. Where to File: If the new commercial enterprise is located, or will principally be doing business in AK, AZ, CA, CO, GU, HI, ID, IL, IN, IA, KS, MI, MN, MO, MT, NE, NV, ND, OH, OR, SD, UT, WA, WI, or WY, mail the petition to the California Service Center.

If the new commercial enterprise is located, or will principally be doing business in any state not listed above, mail the petition to Texas Service Center.
